Terms of use

Agreement

By using the Face On Optical website, online shop, appointment request form, or patient portal, you agree to these terms and our Privacy notice (v2026-09-06). If you do not agree, do not use these services.

Who we are

Face On Optical is an optical shop. Confirm mailing address, phone, and hours on this website (when published) or with clinic staff. Services that require a regulated health professional (for example, comprehensive eye exams) are provided only when a licensed practitioner is available.

Website and content

• Product photos may be category illustrations, not exact images of each SKU.
• Prices, stock, and availability can change without notice until confirmed at the clinic.
• We may update or discontinue parts of the site at any time.

Online shop

Online orders are requests for clinic pickup. Submitting an order does not charge your card on this website. Payment and prescription verification happen at the clinic before or at pickup, unless we tell you otherwise in writing. See our Refund & returns policy.

Appointment requests

Booking forms are requests only. A slot is not confirmed until we contact you. We may suggest another time if your preferred slot is unavailable.

Patient portal

Portal access is limited to your own care information after identity checks and consent. Do not share magic links. Misuse may result in access being revoked.

Acceptable use

Do not attempt unauthorized access, scrape personal data, submit false identity information, or use the site for unlawful purposes.

Limitation of liability

To the extent permitted by Ontario and Canadian law, we are not liable for indirect or consequential losses arising from website use, delayed confirmations, or stock changes. Nothing in these terms limits liability that cannot be limited by law (including for fraud or bodily injury caused by negligence where applicable).

Governing law

These terms are governed by the laws of Ontario and the applicable laws of Canada.

Contact

Questions about these terms: contact Face On Optical using the phone, email, or address shown on this website, or speak with staff in clinic.
